“Network- and state-oriented thalamic neuromodulation for epilepsy”

 

Abstract: Neuromodulation of the thalamus has proven an effective treatment for epilepsy. However, a quarter to half of patients do not achieve significant seizure reduction, and prolonged seizure freedom is rare. Traditional open-loop stimulation ignores the dynamics of epilepsy and seizure risk. Even closed-loop stimulation, based upon instantaneous epileptiform event detection, neglects underlying multi-day cycles of seizure risk and variation in functional connectivity within seizure networks. Our research group works to identify electrophysiologic biomarkers of thalamocortical seizure networks, neural state, and seizure risk to optimize and individualize thalamic stimulation for epilepsy.
